Only CMS-certified facilities from counties in the state of Oregon are available for pre-order at this time. More county reports will become available as we add state reports. Please check back or contact us for an estimate of when reports will be available.
This report is available only for CMS-certified Medicare/Medicaid long-term care facilites (nursing homes). It does not include assisted living facilities, residential care facilities, memory care communities, adult foster homes, Continuing Care Retirement Communities (CCRCs), independent living communities, or any long-term care provider that is not listed in the CMS Nursing Home Care Compare system.
Only CMS-certified facilities are available for a pre-ordered Individual Facility Performance Report.

This report brings together all CMS-certified long-term care facilities located in the selected county and state, and evaluates their collective performance through clear, objective metrics grounded in federal datasets.
